When someone is living with a disability, recovering from an injury, or adjusting to changing healthcare needs, familiar spaces can become difficult to navigate.

Everyday routines such as bathing, resting, entering the home, moving between rooms, or spending time with family may require added effort and support. Caregivers may also face physical strain, ongoing worry, and spaces that no longer fit the household’s needs.

AccessAble is Welcome Home Angel’s program for adolescents and adults managing temporary or long-term health conditions. Through customized home modifications and thoughtful design, we reduce barriers and create surroundings that support safety, function, comfort, choice, and family life.

Every AccessAble project begins by listening.

We learn about the individual, the family’s routines, the challenges within the home, and the changes that would make the greatest difference. The individual and family help guide the project so that each solution reflects how they live and what matters most to them.

Welcome Home Angel then brings together design expertise, healthcare insight, skilled construction, volunteers, and community partners to create a personalized plan.

The result is more than a renovated room. It is a home that reduces daily burdens, restores comfort, and gives families more space to care for and celebrate one another.

Who AccessAble Serves

AccessAble serves adolescents and adults whose homes do not adequately support their healthcare, mobility, accessibility, or daily living needs.

The program may be appropriate for someone who:

• Lives with a physical disability or mobility limitation
• Is managing a chronic or progressive health condition
• Has experienced a serious illness or life-altering injury
• Is returning home following hospitalization or rehabilitation
• Uses a wheelchair, walker, or other mobility equipment
• Needs support with daily routines
• Is transitioning from pediatric care into adult services
• Faces safety or accessibility barriers within the home

Each nomination is reviewed individually. Project approval depends on the household’s needs, the home environment, project feasibility, available funding, and program capacity.

Personalized Solutions for Daily Life

AccessAble does not use a one-size-fits-all renovation plan. Each project is shaped around the individual, the family, and the physical conditions within the home.

Approved modifications may include:

Bathrooms

Roll-in or low-threshold showers, grab bars, accessible fixtures, seating, and layout changes that support safer personal care.

Entrances and Pathways

Ramps, railings, landings, wider doorways, flooring repairs, and clearer pathways for wheelchairs, walkers, and other mobility equipment.

Bedrooms and Living Areas

Adapted spaces that better support rest, personal care, transfers, therapy, studying, working, and time with family.

Equipment and Technology

Transfer supports, specialized seating, adaptive fixtures, safety technology, and other tools selected to address specific needs.

Family Spaces

Changes that make shared areas more comfortable and functional for the individual, caregivers, siblings, and other household members.

These modifications are designed to help the home serve the family, rather than requiring the family to continually adapt to an unsuitable space.

How AccessAble Works

1. Nomination and Application

An individual, family member, caregiver, healthcare provider, case manager, community organization, or friend may submit a nomination. Welcome Home Angel then gathers information about the household, healthcare needs, and challenges within the home.

2. In-Home Assessment

The project team visits the home to understand daily routines, safety concerns, family priorities, and the physical space.

3. Project Planning and Review

The proposed work is reviewed for safety, feasibility, household benefit, available resources, and long-term usefulness. The individual and family remain involved throughout the planning process.

4. Design, Installation, and Follow-Up

Welcome Home Angel works with designers, contractors, skilled tradespeople, volunteers, and relevant healthcare professionals to complete the approved modifications. After completion, the team gathers feedback about changes in safety, function, comfort, caregiver needs, and family well-being.

Support for the Individual and Family

When a home does not meet an individual’s needs, the effects are often felt throughout the household.

Caregivers may spend significant time assisting with bathing, transfers, movement, supervision, and other routines. They may also worry about falls, injuries, emergencies, or whether the current space can continue supporting their loved one.
